We provide specialist assessments, advice, and training for health professionals. 

We work in hospitals and in the community. This includes NHS and private healthcare settings.

Our aim is to:

  • help prevent people from being admitted to hospital when they do not need to be.
  • support people to leave hospital earlier.
  • improve how quickly wounds heal.
  • reduce the number of pressure ulcers.

Our service is available for people of all ages. 

Accessing our service

Referrals to this service are usually made by community nurses, GPs, practices nurses, inpatient wards/ departments or nursing or residential care homes.
