South Nottingham Adult Community Services Integrated Care Team
The team is made up of nurses, specialist nurses, occupational therapists, physiotherapists, assistant practitioners and support workers.
We are a team of health professionals who support adults in the community. Our team includes:
- nurses and specialist nurses
- occupational therapists
- physiotherapists
- assistant practitioners
- support workers
What we do
We provide a range of care and support, including:
- nursing care, such as wound care
- support when people leave hospital
help to prevent falls - end of life care
- continence care
Who the service is for
Our service is for adults aged 18 and over who are unable to visit their GP or practice nurse.
We see people in:
- their own home
- care homes
Supporting independence
Where possible, we help people to stay independent, look after themselves, and return home safely after a stay in hospital.
Support for carers
Supporting carers is an important part of our work. We offer advice and guidance to help carers feel supported.
Accessing our service
Your GP can refer you to this service, or you can contact the Adult Community Hub on 0300 0830 100. The Adult Community Hub is open Monday to Friday from 8am to 5pm.
This service is available in Nottingham North and East, Nottingham West and Rushcliffe.
