We offer specialist assessments for older people with complex and high risk mental health needs. 

Our team supports people to live as independently as possible. We believe everyone with mental health problems has the right to high quality care. 

We provide care in people's own homes whenever we can. We work hard to avoid unnecessary stays in hospital or residential care. 

The IHTT team offers intensive support in the patient's home for up to 8 weeks. This support helps improve mental health and supports recovery.

Accessing our service

The service provides support under the following eligibility criteria:

  • The patient must have a GP in  the area the service is being delivered. 
  • Be aged 65 or over with complex mental health difficulties
  • The service also sees patients of any age with working age dementia
  • People living alone who are at risk of self-neglect due to their mental health or are a danger to themselves or others

Service opening hours

The service is provided in patient's own homes between 8am to 8pm, 7 days a week, 365 days a year.
