Dementia Outreach Service (County)
We provide mental healthcare support to people of any age who have a diagnosis of dementia and live in a residential care home or care home with nursing.
We work to improve the quality of care for people with dementia who live in care homes.
We offer specialist assessment and support for people of any age who have a diagnosis of dementia and live in a residential care home or a care home with nursing.
The service is available across Nottinghamshire County. A separate service is available for people who live in Nottingham City.
Service opening hours
Monday to Friday from 8:30am - 4:30pm.
Accessing our service
The service can be accessed via the Older People's Community Mental Health Team in each area.
Written referrals are accepted from your GP, community matrons, adult social care and health, continuing healthcare team, mental health intermediate care teams and direct from care home managers.

Tram
The Highbury Vale Tram Stop is a 10 to 15 minute walk from the hospital.

Highbury Hospital offers free onsite parking for both visitors and outpatients.
Spaces can be limited after 9am. Do allow a little extra time to find a parking space, especially during busy periods.
